All Loan requests must be accompanied by the following:

1. Completed Midwest Loan Fact Sheet.
2. Last three years financial statements and most recent year-to-date interim statements for the proposed security, the borrowing entity and any proposed guarantor, co-borrower and/or general partner.
3. Last three years' tax returns for the borrower, guarantor and/ or general partner.
4. Most recent comprehensive rent-roll.
5. Copy of all major tenant leases.
6. Site plan.
7. Area map.
8. Photographs.
